Directions: 

Click on the link below to open the Grant Summary Sheet as a Word document.  At the File Download box, click Save and save the file to your hard drive.

Complete the form by clicking in the center of each box to add text or a check mark.  Click the Save button on the toolbar to save the data that you entered into the form fields.  Click the Print button on the top bar to print the form.
